Social Determinants of Health What are they?
How do SDOH relate to Kidney Transplant?
According to the CDC, Social Determinants of Health are non-medical factors that affect health outcomes. They include conditions in which people are born, grow, work, live, and age. (CDC, 2024)
Why are SDOH important when it comes to kidney transplant? (CDC, 2024)
- Addressing a person’s SDOH can enhance their health and lead to better outcomes when it comes to the overall well-being a person (physical, mental, emotional)
- Assessing SDOH can uncover underlying social factors that can interfere with a patient’s success pre/post kidney transplant.
- By assessing SDOH for patients, patients and providers can work with one another to meet the need of a patient such as providing necessary resources, education, and identifying necessary support.
